Position in Labour and Delivery: Based on the best available research evidence

Interest in maternal position during labour is not new, but only recently has there been any well controlled research to access the validity of the various strongly held opinions.

At the present time lying down in labour is common in many maternity units, with 74% of women in the world adopting a semi recumbent position (1). The available data cast doubt on the wisdom of this practice (2)

Applying Evidence-based Advocacy in a Decentralized Context
In 2001, Indonesia’s administrative and financial powers of government were rapidly devolved to 550 districts, in what is now described as the “Big Bang Decentralization”. This mass decentralization included budgeting for health services and supplies. In the intervening years, district governments have struggled to identify which health issues to prioritize and the level of funds needed to achieve improved health outcomes.

In addition, the National Family Planning Board’s (BKKBN’s) role in championing the value of family planning as a health intervention diminished at the local level. Without adequate evidence and expertise, commitment and resources to promote and provide access to family planning dwindled.

So, The project (KB) must rekindle local commitment to family planning and reengage champions, expanded to all of the district, aims to increase access to quality family planning services among low- and middle-income populations, including marginalized groups and communities living in remote villages.


Fewer Women Die When Pregnancies are Planned
Family planning (KB)  saves women’s lives in two ways. First, family planning helps women avoid  unwanted pregnancies. Each year, 687,000 maternal deaths are averted with family planning  versus the 533,000 maternal deaths that occur (red: Personal communication with Amy Tsui, Professor, Johns Hopkins School of Public Health and Director, Bill and Melinda Gates Institute for Population and Reproductive Health. Trends in Maternal Mortality:1990 to 2008. Estimates Developed by WHO, UNICEF, UNFPA and the World Bank, Geneva: WHO, 2007) Women who are not pregnant do not die from pregnancy-related causes.

Latar Belakang
Untuk mewujudkan bangsa yang berdaya saing sesuai dengan arah pembangunan jangka panjang yang tercantum dalam Undang-undang Republik Indonesia Nomor 17 tahun 2007 tentang Rencana Pembangunan Jangka Panjang Nasional/RPJMN tahun 2005-2025 perlu adanya usaha mengedepankan pembangunan sumber daya manusia, yang ditandai dengan meningkatnya Indeks Pembangunan Manusia (IPM). Unsur-unsur penting bagi peningkatan IPM adalah derajat kesehatan, tingkat pendidikan, dan pertumbuhan ekonomi. Derajat kesehatan dan tingkat pendidikan pada hakikatnya adalah investasi bagi terciptanya sumber daya manusia berkualitas, yang selanjutnya akan mendorong pertumbuhan ekonomi dan menurunkan tingkat kemiskinan.

Hal ini didukung oleh adanya Keputusan Menteri Kesehatan Republik Indonesia No 564/MENKES/SK/VIII/2006, yang menyebutkan Desa/Kelurahan siaga aktif adalah Desa/Kelurahan yang memiliki kesiapan sumber daya dan kemampuan untuk mencegah dan mengatasi masalah-masalah kesehatan, termasuk diantaranya adalah bencana dan kegawatdaruratan kesehatan secara mandiri. Tujuannya adalah terwujudnya masyarakat desa/kelurahan yang sehat, peduli dan tanggap terhadap permasalahan kesehatan di wilayahnya.

Pelatihan ToT Konselor Pemberian Makanan terhadap Bayi dan Balita (PMBA)




PEMBERIAN MAKAN PADA BAYI DAN ANAK (PMBA)

Dinas Kesehatan Prov. DKI Jakarta beserta Suku Dinas Kesehatan Kota Administrasi Jakarta Barat dan Mercycorps Indonesia, telah melakukan pelatihan konselor dan pelatih konselor untuk PMBA. Pelatihan PMBA dirancang untuk membekali petugas kesehatan atau kader kesehatan di tingkat masyarakat dan kelurahan/desa hingga provinsi dalam membantu para ibu, ayah dan pengasuh agar dapat memberikan makanan kepada anaknya dengan optimal dan benar, disertai dengan pengetahuan teknis tentang cara pemberian makanan yang direkomendasikan untuk ibu hamil dan anak usia 0 hingga 24 bulan, meningkatkan ketrampilan konseling, pemecahan masalah dan negosiasi (mencapai kesepakatan) serta memanfaatkan alat bantu dan alat konseling secara efektif.

Mercycorps in Indonesia has strategies, which to improve community infrastructure, health, resiliency and economic opportunities in Indonesia’s most challenging urban and coastal areas. About half of all Indonesians live on less than a dollar a day. Employment growth has been slower than population growth. Public services remain inadequate by middle-income standards, and health indicators are poor. Our work, in economic opportunity, we provide technical assistance, training and financial services to microfinance institutions throughout the country. In health sector, Mercycorps Indonesia raising awareness and supporting mothers to practice and promote exclusive breastfeeding, addressing childhood malnutrition through healthy, affordable food carts in every kind of place and improving sanitation and hygiene in crowded urban areas with a mobile sludge removal service. We also work in disaster preparedness, which is identifying and mapping areas at risk and helping those communities plan, train and practice how to respond when disasters occur, and maintaining a response team ready to quickly deploy and provide immediate relief to survivors during the critical first months after a disaster strikes. 
 


Since 2009, exclusive breastfeeding is an acknowledged and protected right of every mother and baby in Indonesia, but today still about 4 out of every 10 children under five years old are stunted, according to Basic Health Survey in 2013. In Jakarta Province (DKI Jakarta), which has 5 municipal city and 1 region, 1 of 3 in the same age group have grown up shorter than their peers. Despite the growing awareness that exclusive breastfeeding for six months continued with breastfeeding and adequate source food through the first two years of life would optimize growth and development of children to their full potential, based on Lancet 2006, the practice of exclusive breastfeeding (EBF) are shadowed by significant raise in infant formula milk factory marketing and bottle feeding over the years.

Masih teringat dalam ingatan kita bahwa hampir berhari-hari dalam dua bulan Januari dan Februari di awal tahun 2014, ibu kota negara tercinta ini diguyur hujan deras, sehingga di layar TV kita pun ditutupi dengan berita tentang “Banjir Ibu kota Jakarta”. Tidak hanya ibu kota yang terkena bencana tapi juga daerah lainnya, seperti air bah di Menado, letusan Gunung Sinabung, letusan Gunung Kelud, dan banyak lagi di tahun 2014 ini maupun sebelumnya. Sebagai warga yang tidak terkena bencana tentunya kita tidak tahu pasti keadaan dan perasaan mereka yang membutuhkan pertolongan, kecuali kita hanya bisa berdoa dan membantu segenap daya dan upaya.
